TWIN FACTS
 

  • They are Monochorionic diamniotic (Mo-Di) twins, which are identical twins that share a placenta, but each have their own amniotic sac. 

  • One has Selective Fetal Growth Restriction (sFGR), a condition that occurs when one fetus in a monochorionic twin pregnancy doesn't grow as well as the other. The placenta is not evenly apportioned between the twins. sFGR is classified into three types, Type 3 is present for one of the girls.  

  • Type 3 presents an unpredictable pattern of intermittent blood flow in the umbilical artery of the growth-restricted twin (forward, absent and reversed). The blood flow in the umbilical artery of a growth-restricted twin fluctuates between heartbeats.

    • Blood Flow Pattern: The blood flow in the umbilical artery is intermittently absent, reversed, or forward. This pattern is unique to monochorionic twins with sFGR.

    • Risk: It's associated with a higher rate of neuro impairment, fetal death, and fetal brain injury.

    • Delivery: The average age of delivery is 30–32 weeks gestation.

    • Monitoring: Frequent ultrasounds and fetal echocardiograms are used to monitor the babies' growth, amniotic fluid, and blood flow.

PROGRESS UPDATES
 

16 WEEKS

Nov 11, growth ultrasound. Babies are now 2.8 pounds and 2.10 pounds. Delivery is officially scheduled for 32 weeks out and we are on the countdown. They are about the size of a coconut. 

Oct 29 - Nov 10, twice a week ultrasounds. Heart monitoring for the twins 3 times a day for a minimum of 40 min. 2nd round of steroid shots administered. Glucose test taken 3 times, levels were ultimately high. So now, blood sugars are tested first thing in the morning and after every meal by finger prick.  Numerous other tests, shots, and blood draws.  

Oct 28, went for a follow up visit. There had not been any improvement with the blood flow patterns, so it was recommended I be admitted until delivery. Doctors planned to continue to monitor and anticipate delivery to be no later than 32 weeks, but could be earlier depending on the monitoring.

Oct 21, went in for a routine visit. Due to concerns with blood flow for the smaller twin seen during the ultrasound I was admitted for 48 hours for closer observation. During hospitalization I had to get steroid shots for the twins to help with further development of their lungs as they will be delivered preterm. Oct 23, released from hospital.

 

Referred to Seattle Children's Fetal Care and Treatment Center for weekly ultrasounds to monitor growth and the blood flow.
